My name is Monica Coulter, and I am a Licensed Professional

Counselor, license number #68705. I am also a Licensed

Professional Counselor Supervisor (LPC-S) and am able to assist

           LPC Associates.

           What I provide: I provide in-office and virtual counseling/therapy

           to adults ages 18+.

What to expect: We will first schedule an initial intake session,

which is a great way for us to meet face to face and see if I am

the best match for you. Research has shown that one of the most

important aspects of therapy is the therapeutic relationship

and building a strong rapport. Feel free to be yourself and come

as you are, as I enjoy doing the same. I strive to provide a laid back

atmosphere where we can both be ourselves. My office is a

Safe Zone to those who self-identify as LGBTQIQ, polyamorous,

their families, and their partners/spouses. If we decide to continue

with the counseling process, you can schedule appointments

           accordingly.

           Appointments: Available times include Tuesday-Thursday

           from 10:00-7:00, and Friday-Saturday from 10:00-3:00.

Fees: The first initial intake session is $120.00 and is scheduled

for 80-90 minutes. Each subsequent session is $120.00 per session

and is scheduled for 50-60 minutes. Payable by check, cash, or

credit card at the time of the session.

Insurance: I am an in-network provider with Blue Cross

and Blue Shield, Cigna, and Baylor Scott and White.

I am an out-of-network provider for all other insurance companies,

and I can provide you a superbill for you to give to your insurance

company for partial or full reimbursement.

Appointment reschedule: If you cannot attend a scheduled

appointment, please notify me at least 24 hours in advance

so I can reschedule your appointment.

**In the event of an emergency, call 911 or go to a local emergency

room when necessary and appropriate. It is your responsibility

to seek the appropriate resources in emergency situations.
